Dear Caregiver. Please read the important information below before filling out the answers to the caregiver sign-up questionnaire. The answers you provide will determine your eligibility to be paired with a potential patient.
Before the potential patient is able to view members of our caregiver network, he/she will be asked to answer two questions:

1. What is your zip code?
2. What kind of caregiver service type do you require?

The answers provided will potentially eliminate many caregivers from being eligible. For example, if the zip code that is provided is a greater distance (in miles) than you listed you are willing to travel, you will be eliminated. If the service type chosen, does not match the service type(s) that you provide, you will be eliminated.
Below is information on the definitions of the various service types, so that you will be able to answer the questions appropriately, if you would like to provide that service type.

There are 5 categories of "Service Type" into which a caregiver can belong. A caregiver can belong to multiple categories as well.

Caregiver Service Type Definitions/Criteria:
  • On-Demand Caregiver
    This means that you are available on short notice to provide caregiver services. See Criteria
  • Scheduled Caregiver
    This means that you typically do not have set hours every day that you are able to provide services. See Criteria
  • Part-Time Caregiver
    This means that you are looking to provide services for 20 hours a week with the same set hours each day. See Criteria
  • Full-Time Caregiver
    This means that you are looking to provide services for 40 hours a week with the same set hours each day. See Criteria
  • Live-In Caregiver
    This means that you are looking to move in with the client and provide services around the clock. See Criteria
